Card4Life is a USB device that plugs into any computer running Windows XP and higher which has a built-in program that allows you to enter your health information. It is password protected so the information entered is as secure as the cardholder desires. However, your social security number and insurance policy numbers will always be secured to help prevent identity theft. Secured information can ONLY be accessed by use of the password you as the cardholder enter.

The "pages" of information you enter are important for an accurate picture of your past and current health. It is recommended that a blank copy of the form, provided as a part of your Card4Life, be printed, completed by hand and then entered into the Card4Life program. We also recommend the completed document be reviewed by your personal physician and any specialists to insure the medical information entered is accurate and complete. A complete set of instructions are supplied with your Card4Life upon delivery.
The development of Card4Life was done in consultation with medical professionals, including physicians, hospital administrators, nurses, EMT personnel, as well as end users.
Not only is the Card4Life program installed on the USB device, there is plenty of additional space for other important medical information such as a Living Will, Medical Power of Attorney, digital x-rays and any other documents you might want to make available to medical personnel, attorney's and family.
Your medical information can and should be updated when there is a change in your medical status. Please remember, Card4Life can be a terrific time saver if you change physicians or specialist as your medical information can be printed out and handed to your new doctor. If your doctor's office desires they can simply add the report to your electronic medical chart by copying the report to their computer file.

Carry Card4Life with you and in the event of an emergency. Allowing emergency medical staff access to your basic medical information can be a life-saver. By allowing them instant knowledge of what medications you are taking, along with any possible drug interaction between your medication and emergency administrated meds, or any medication allergies could mean the difference between life or death!
Card4Life is SECURE! You have the option of making as much or as little of your information password protected. Any information you protect with a password will NOT be available to someone who picks up your device and plugs it into a computer. However, to help you in case of an emergency your first name and date of birth is available to emergency services staff along with your emergency contacts first name and telephone numbers. Your social security number and insurance policy numbers are always protected even if you leave the rest of your information unprotected.
There is no need to connect to the internet with Card4Life as your medical information is not stored on a computer server somewhere that a hacker could get into and steal your information. You carry it, you protected it!
